Services are by appointment only. My hours of operation vary according to the service being rendered. I am available at different times Monday-Friday for psychotherapy sessions. I am generally available on Saturdays for primarily non-psychotherapy related services. I provide telehealth or other virtual services only with few exceptions such as Hoarding which requires my presence at the physical location of the hoarding, some exposures, and some business workplace sessions. I intend to reply within 24-48 business hours.
If you have questions regarding anything pertaining to psychotherapy services such as treatment interventions or therapeutic approach please review my website in its entirety as most questions should be answered from the information made available. I’m happy to speak with potential clients briefly if additional questions remain unanswered such as cost of service or how to set up an appointment with me. Notice of Privacy Practices
This document contains HIPPA rights and responsibilities and general practice guidelines which includes billing, forms of payment, range of fees relative to service, contacting me, medical records, and telemedicine guidelines. Your signature is requested, and the last page is returned to me upon reading this document in its entirety, preferably prior to our first session. This document represents our contract.
Demographic Form
This document contains basic demographic information that should be filled out prior to our first session and returned to me. Please include an emergency contact person and corresponding contact number which may be used in case of an emergency. You are giving permission for me to reach out to this person under the aforementioned circumstance so consider your selection carefully.
Credit Card Payment
This document is used only for clients electing to pay by credit card and not Venmo. A 4% fee is added to your total balance for using this form of payment. Please fill out this form. This form can be faxed to me at a confidential location and number per the client’s preference: (914) 637-3602. We can discuss alternative methods of transferring this document to me in the interest of maintaining sensitive information.
